Help leaving hospital
Social services provide an intermediate care service to help people bridge the gap between leaving hospital and returning home. Intermediate care is designed to help people get back on their feet and increase their confidence to tackle day-to-day tasks such as making meals, bathing or just generally getting around without help or with minimal assistance. Contact your Social Service office for further information (details below).
Additional information: For short-term help at home, following discharge from hospital, the British Red Cross provide the Home from Hospital scheme. They also offer short-term practical help to people faced with an unexpected crisis if, for example, a carer is taken ill.
